1. slang To speak truthfully and share everything one knows or feels about something, especially a sensitive issue or a wrong one has committed. After some questioning by police, the man eventually spilled his guts on his involvement with the crime. She started spilling her guts on how nervous and uncertain she was about getting married.
2. slang To be completely and perhaps overly candid in telling something to someone. I'm sorry for spilling my guts on you like that. I just haven't had anyone to talk to since the divorce. Most people don't like it if you spill your guts on them after they casually ask you how you are doing.
3. slang To vomit violently and extensively onto something. He got so drunk at the bar that at one point he turned and spilled his guts right on the floor. Somebody at the party must have spilled their guts on my sofa, because it absolutely reeks of vomit.
