To be puzzled, doubtful, or uncertain; to be having trouble understanding something. Literally scratching one's head can be a sign that one is confused about something. They're all scratching their heads over his decision to pull the plug on the lucrative program.

COMMON If you are scratching your head, you are puzzled and unsure about something. The viewer is left scratching his head as to why Brown should have gone to all the trouble. Relief workers outside were scratching their heads about what to do next. A lot of people are scratching their heads and saying, `What are we doing? Are we getting our money's worth?' Note: You can also talk about head-scratching. There had been much head-scratching among BBC executives who hadn't then thought up a suitable name for the show.
