To be mentally unwell or unstable. Possibly offensive; often used hyperbolically. Tim has been behaving very erratically lately. I'm afraid that he's messed up in the head because of the accident. Oh, Liz is messed up in the head if she thinks that plan will work! You can't tell people you want to start a business selling bees as pets—they'll think you're messed up in the head!
