To be something that one is personally responsible or to blame for. You chose to break the law, so it's on your head if you get arrested for it. No, I accept the consequences. I knew it would be on my own head if we got caught trying to sneak out. The boss knows you're in charge of this account, so it's on your head if we lose it!
