To cause one an unbearable amount of grief, sadness, or distress. It tore my heart out to see Billy in so much anguish, knowing that there was nothing I could do to help. It was plain to see that the death of Rob's wife had torn his heart out. Can't you see you're tearing Jane's heart out? Why do you have to treat her that way?
