1. Bent in such a way that something's typical shape or form has been lost. There's definitely a dislocation here—her leg is completely out of shape. The box got smushed a bit out of shape during transit, but everything inside was fine.

2. Not or no longer physically fit. Oh man, I'm so out of shape—I get winded just walking up a flight of stairs! I've gotten pretty out of shape after working behind a desk for so many years.

not in good physical condition. I get out of breath when I run because I'm out of shape. Keep exercising regularly, or you'll get out of condition.
